Federal Government 30% Rebate Form
Complete this application to receive the reduction for your contributions. Please note the Rebate will not be reflected on your membership until we have received this application form.
Health Management Authorisation Form
Under GU Health's Health Management benefits, members can claim towards the cost of an accredited or recognised program designed to address or improve a specific health or medical condition. Ask your GP, medical specialist or recognised healthcare provider to complete and sign this form prior to commencing treatment.

Important Information Booklet
The Important Information booklet forms part of your membership policy and should be read carefully and retained for future reference. It includes information on making claims, using your HICAPS card, answers to frequently asked questions, useful definitions plus information on private health insurance products. Important Information booklets may vary depending on your level of cover. To ensure that you have access to the information that is right for you, request a booklet from your Member Relations Team or download the Important Information Booklet.
