Provider Registration Application
With this for m you can apply to become a GU Health Registered Provider
Claim form
You can simply print out a claim form, fill in the details and fax it to us on 1800 656 778.
Federal Government 30% Rebate Form
Complete this application to receive the reduction in your contributions. Please note the Rebate will not be reflected on your membership until we have received your application form.
Transfer Certificate Request Form
To allow GU Health access to information on your previous health cover, please complete the Transfer Certificate Request form and forward to us.
Fastback Form
With this form you can nominate an account into which your claim benefit payments will be paid.
Third Party Access Form
With this form you can nominate a person to have access to your membership.
Register a Student Dependant
With this form you can register a Student Dependant so that he or she
remains covered by your Family Membership.
